The Troup Cares Free Clinic provides a large range of services to our patients.
Patients come to the Troup Cares Free Clinic at the onset of medical issues and are able in most cases to correct them at that time. This eliminates costly trips to the Emergency Room and potential hospital admissions.
Care Management / Patient Education
The Troup Cares Free Clinic Care Management program is outstanding. A full time Care Manager / Registered Nurse is able to work with each patient and outline an individual program of care for each.
The Care Management Program covers everything from proper diet, proper prescription drug usage, how to refill prescriptions at the proper time along with a never ending list of daily issues that never seem to stop.
A nurse practitioner is available each day to see patients and work within the care management framework thus providing the best medical care and management possible.
Prescription Assistance Plan (PAP)
A Care Manager works with each patient where necessary to help them obtain much needed and in many cases expensive pharmaceutical drugs from the various suppliers and governmental agencies. A team monitors prescription drug usage and timing for resupply to make sure that our patients have adequate medication on an ongoing basis.
Specialists for Appropriate Medical Referrals
Volunteer referral practitioners provide specialty referral services on an ongoing basis.
Standard Social Services
Troup Cares has access to many Social Services that are not necessarily in the normal flow of clinic operations. Patients are directed to areas for additional assistance in areas of non-medical programming.
Troup Cares exists to identify opportunities, seek solutions, and organize community resources to improve access to health services resulting in a healthier and more economically viable Troup County.
